1. Who this notice is from

Dr Chang Fu Shi is a senior dental surgeon registered with the Singapore Dental Council (SDC No. [TBC]), practising at T32 Dental Group, Camden Medical Centre, One Orchard Boulevard, #17-00, Singapore 248649.
