PDPA Notice

Effective date: 26 April 2026 Last updated: 26 April 2026 Version: 1.0
What this is This notice is issued under the Singapore Personal Data Protection Act 2012 (PDPA). It sits alongside the Privacy Policy and sets out the specific obligations the PDPA places on Dr Chang Fu Shi when personal data is collected, used, or disclosed in the course of this website and the enquiries it supports.

1. Who this notice is from

Dr Chang Fu Shi is a senior dental surgeon registered with the Singapore Dental Council (SDC No. [TBC]), practising at T32 Dental Group, Camden Medical Centre, One Orchard Boulevard, #17-00, Singapore 248649. Dr Chang is the organisation responsible for personal data collected through the website at drchangfushi.com for the purposes of the PDPA.

2. Data Protection Officer

Under the PDPA, every organisation must designate a Data Protection Officer. For the personal professional website of Dr Chang Fu Shi, the Data Protection Officer is Dr Chang herself, reachable at:

Dr Chang Fu Shi (Data Protection Officer)

c/o T32 Dental Group

Camden Medical Centre, One Orchard Boulevard

#17-00, Singapore 248649

Phone: +65 6733 1388

WhatsApp: +65 9830 6990

3. Purposes for which personal data is collected

Personal data is collected, used, and disclosed for the following purposes. By phoning or messaging the clinic, or by continuing to use the website, you are treated as consenting to these purposes unless you tell us otherwise.

  • To respond to enquiries about services available at T32 Dental Group.
  • To arrange and manage appointments with Dr Chang at the clinic.
  • To run the website reliably, investigate technical issues, and protect against abuse.
  • To comply with legal obligations, respond to lawful requests, and defend legal claims.

4. Deemed consent and notified consent

The PDPA allows reliance on deemed consent when you voluntarily provide personal data for a purpose that is obvious in the circumstances, such as giving your name and phone number to request an appointment. Where notified consent is relied on, the purpose will be stated clearly and you will be given a reasonable opportunity to opt out. For any use that falls outside the purposes listed in section 3, fresh consent will be sought.

5. How to withdraw consent

You may withdraw consent for the collection, use, or disclosure of your personal data for any purpose at any time, by giving reasonable written notice to the Data Protection Officer at the contact details in section 2.

Once your withdrawal is received, the likely consequences will be explained. Withdrawal may mean that services can no longer be provided to you through this channel. The request will be given effect within a reasonable time, usually within 30 days, and no charge will be made for withdrawal. Withdrawal does not affect lawful processing that has already taken place.

6. Access and correction

You may ask the Data Protection Officer to:

  • Provide a copy of personal data held about you, together with information about how it has been used or disclosed in the past 12 months.
  • Correct any personal data about you that is inaccurate or incomplete.

Identity may need to be verified before the request is fulfilled. The PDPA allows a reasonable fee for access requests; any fee will be notified to you in advance.

7. Transfer of personal data outside Singapore

Some service providers that support the website (hosting, font delivery) process data outside Singapore. Where personal data is transferred overseas, we rely on contractual and operational measures that secure a standard of protection comparable to the PDPA, including written contracts binding the recipient to protective obligations.

8. Accuracy and retention

Reasonable effort is made to keep personal data accurate and complete, particularly where it is used to make a decision that affects you. Please keep your contact details up to date so that communications reach you correctly. Personal data is retained only for as long as the purpose for which it was collected remains valid, or for the duration of any retention period required by law. See section 6 of the Privacy Policy for specific retention periods.

9. Protection obligations

Reasonable administrative, technical, and physical security arrangements are maintained to protect personal data, proportionate to the limited scope of data handled through this website. Clinical records created at T32 Dental Group are covered by the clinic's own information security measures.

10. Complaints

If you believe your personal data has been handled in a way that does not comply with the PDPA, please contact the Data Protection Officer first so the matter can be addressed. If you are not satisfied with the response, you have the right to lodge a complaint with the Personal Data Protection Commission of Singapore:

  • Website: pdpc.gov.sg

11. Changes to this notice

This PDPA Notice may be updated from time to time. The "Last updated" date at the top will reflect the most recent revision. Where fresh consent is required, it will be sought before any change is applied to you.
